The parent or legal guardian as indicated in
the area below as "Parent’s or Legal Guardian’s Name", of the registrant’s
name as indicated in the area below as "Player’s Name", hereby consent to
the participation by said registrant in the Ozarks
Football League for this year. In consideration of the permission granted
by said registrant to Ozarks Football to participate in its sports activities, the individual indicated in the area below as "Parent’s or Legal Guardian’s Name" hereby releases and discharges Ozarks Football League and the Springfield School Board, the City of Springfield and all other municipalities where the program is conducted (and their
respective agents, employees, officers and directors) from all claims,
demands, actions, judgements and executions which the individual indicated
in the area below as "Parent’s or Legal Guardian’s Name" (or their heirs,
executors, administrators or assigns) ever had, now has, or may have, or
claim to have, against Ozarks Football League or the Springfield School Board,
the City of Springfield and all other municipalities where the program is
conducted or their respective agents, employees, officers and directors,
either on their own behalf or on behalf of said registrant, for all
personal injuries, and injuries to property, sustained by said registrant
as the direct or indirect result of his or her participation in the above
described sports activity.
The individual indicated in the area below as "Parent’s or Legal
Guardian’s Name" acknowledges that they are fully aware of the obvious,
necessary and inherent risks of this sport, and we represent to
Ozarks Football League that we, and the below named registrant, recognize and
fully appreciate the risks inherent and related to this activity. Further, the individual indicated in the area below as "Parent's or Legal Guardian's Name" fully understands that the purpose of Ozarks Football League is to teach and instruct the sport of football and the below named registrant may or may not play in a competitive game conducted by the Ozarks Football League.
The individual indicated in the area below as "Parent’s or Legal
Guardian’s Name" further represent to Ozarks Football League that this
registrant is physically able to participate in this program. Parent or Legal Guardian assumes
full responsibility for the determination of registrant’s physical
condition. The applicant/participant has received a physical examination by a physician and has been found physically capable of participating in the programs.
I HEREBY AUTHORIZE THE BELOW NAMED REGISTRANT TO PARTICIPATE IN THE
OZARKS FOOTBALL LEAGUE ACTIVITIES.
I hereby certify that the information provided on this form
is true and correct and any false information submitted may result in the dismissal of the applicant/participant
without refund. I agree to pay $200 for any rental equipment not turned in by season's end.
